Alcohol, Nicotine, and Caffeine After a Hair Transplant: The 14-Day Blood Flow Rules
A new graft does not lock into the scalp on surgery day. For several days it sits in a small opening, held by fluid and a fragile clot, while new blood vessels grow toward it. Crusts often loosen between day 10 and day 14. Alcohol, nicotine, and caffeine all change blood flow, clotting, or sleep in that window.
These notes follow the timing iGraft uses in its written instructions. If your sheet is stricter, follow the sheet. At the clinics in Pune, Bangalore, Hyderabad, and Vijayawada, the team confirms your plan before the date is fixed.
Why the first 14 days are a blood-flow problem
Transplanted follicles do not arrive with a finished blood supply. Oxygen has to reach them through vessels that form over the first two weeks. Through the first 10 days, the graft is mostly held by clot and fluid. Those same two weeks are when new vessels grow in and crusts start to fall.
Nicotine narrows small vessels. Alcohol can thin the blood. Caffeine is milder, but it can raise blood pressure and disturb the sleep you need while swelling peaks, often from day two to day five.
A quiet two weeks protects work already done. New hair does not show in week one. Later shedding of transplanted shafts is a known part of the cycle. The myths page explains that timing so you do not treat normal fallout as a lost graft.
Nicotine: the strictest pause
Nicotine is a vasoconstrictor. Cigarettes, cigars, vapes, and chew all count. Reduced flow can slow healing, raise infection risk, and in some cases affect graft survival.
iGraft asks you to stop at least 24 hours before surgery and to stay off tobacco for at least one week after. That week is a minimum, not a target. Other teams advise two weeks off before surgery and two to four weeks after, or several weeks in total. Use 14 days as your floor after the procedure. A month is better if you can hold it.
Patches and gum still deliver nicotine, so ask before you swap. Do not restart on day eight because the scabs look dry. The vessels under them are still new.
Alcohol: clotting and swelling
Alcohol reduces clotting. Sites can bleed more, forehead and eye swelling can worsen, and grafts settle more slowly. It also dehydrates you and may clash with post-op medicines. Many teams treat the first 7 days as strict abstinence, and they treat day 10 as the point when grafts are usually more secure.
Before surgery, iGraft recommends three dry days, with one week as the better aim. After surgery, published minimums range from 24 hours to 7 days, and some teams use 10 to 14 days. Skip beer, wine, and spirits through day 14.
If a medicine already thins your blood, do not add alcohol. Do not restart aspirin unless the prescribing doctor agrees. Tell the iGraft doctor what you take. Diabetes, blood pressure treatment, and a very young age need a review before you are cleared. The procedure can be safe in a screened patient. It is still surgery.
Caffeine: none on surgery morning, light after
Skip coffee, tea, energy drinks, and caffeine tablets on the morning of surgery. Eat breakfast and leave the coffee. A 24 to 48 hour cutback beforehand is a sensible buffer, because caffeine can raise heart rate and blood pressure while you are in the chair.
A small amount after the first two or three days is reasonable for many people. Sleep with your head raised, and skip a late cup while swelling is peaking. Wait for heavy intake until scabs shed, often 7 to 10 days. After the first week, ordinary coffee is not clearly tied to graft loss, though some teams still prefer seven full days off. If your sheet is stricter, follow it.
Keep shampoo, serum, and other products off the grafts until washing is cleared. An iGraft shampoo or serum can wait. Early rubbing does more harm than a missed wash.
The 14-day rules at a glance
| Item | Before surgery | Days 1 to 7 | Days 8 to 14 |
|---|---|---|---|
| Nicotine | Stop at least 24 hours, ideally 2 weeks | None | None, then stay off longer if you can |
| Alcohol | 3 to 7 days off | None | None through day 14 |
| Caffeine | None on surgery morning | Light only if cleared | Normal intake once scabs shed |
| Exercise | As usual, unless told otherwise | Rest and short walks | Light cardio if cleared, no heavy lifting |
| Graft contact | Not applicable | Do not touch | Wash only as shown, no rubbing |
Skip saunas, steam, and sweaty workouts for at least a week. Heavier lifting generally waits until day 14. Heat and a tight cap both bother new grafts.
Same pause, either method
ActiveFUE and Gen3 DFI Ultimate (Direct Follicle Implant) both move living follicles. The substance rules do not change with the method. A graft is the tissue unit taken from the donor area. One graft may hold one to four hairs. That number shapes density. It does not shorten these 14 days.
A no-shave plan, when your donor area allows it, does not shorten the pause. A beard transplant follows the same clock. iGraft matches the method after a computerized follicular scan, not from a photo. When donor hair is enough, surgery can help pattern baldness. Moved follicles are a long-term result, not a same-week change, and not every case succeeds.
If surgery is not the first step, PRP or GFC may be discussed after the scan. Those sessions have their own calendar. They do not replace the blood-flow rules after a hair transplant. Six-month follow-up is part of that care, so nicotine and alcohol questions do not end on day 14.
FAQ
When can I smoke after a hair transplant?
Not for 14 days. iGraft's minimum is one week. Longer is safer. Vaping, cigars, and chew count the same as cigarettes.
When can I drink alcohol or coffee?
No alcohol for 14 days, and three to seven dry days before surgery. No caffeine on surgery morning. Light coffee after a few days is often fine. A normal amount waits until scabs shed, around day 7 to 10.
Why 14 days, not 48 hours?
The graft is still relying on a clot and on vessels that are only just forming. Crusts commonly loosen between day 10 and day 14. Feeling fine does not mean the blood supply is finished.
Can I exercise, use a sauna, or restart aspirin?
Short walks are fine. Skip saunas, steam, and sweaty workouts for at least a week. Heavy lifting usually waits until about day 14. Do not restart aspirin unless the doctor who prescribed it agrees.
Is it safe, and does it work?
In screened patients it is a standard procedure. Local anesthesia controls sharp pain, and soreness after is normal. It can work for pattern baldness when donor hair is enough. The moved hair is usually lasting, not instant, and not guaranteed in every case. Your doctor should say that before you book.
Get the plan in writing
If you are deciding when to get a hair transplant, start with the scalp. Take the two-minute quiz or book a consultation at an iGraft clinic in Pune, Bangalore, Hyderabad, or Vijayawada. You meet the doctor in person. Ask about nicotine, alcohol, and coffee before the date is fixed, and bring your medicine list. The written sheet is the rule you follow.
