You can relocate hair. You cannot grow a second donor supply. The 50 percent donor threshold is a lifetime ceiling, not a one-day quota. Cross it, or harvest zones that will later bald, and the back and sides can look mottled or see-through.
If you are weighing clinics in Pune, Bangalore, Hyderabad, or Vijayawada, or flying from the US, UK, or Australia, settle this math before you compare techniques or prices.
What 50 percent does and does not allow
Bernstein and Rassman noted that the safe donor is about a quarter of the scalp, and that up to half the hair there can be taken while apparent density still looks similar. A study of 580 Indian men found a mean scalp density of 78.2 follicular units per cm2. They used 25 percent in the first sitting, then 25 percent of what remained at least six months later, for a lifetime total near 44 percent. Taking half at once raises the chance of microvascular damage. The tables are on pmc.ncbi.nlm.nih.gov.
The ISHRS forum prefers residual density. Typical baseline is 65 to 85 follicular units per cm2, and many surgeons want 40 to 50 left. A safe zone of 189 cm2 at a baseline of 65 can spare 10 to 15 units per cm2, about 1,890 to 2,835 grafts, and still leave 50 to 55. A 3,000 to 5,000 graft plan on that donor can push residual under 50. See-through shows sooner with fine, straight, short hair. The example is on ishrs-htforum.org.
Half of a baseline of 100 still leaves 50. Half of 70 leaves 35. A percentage without a density count is guesswork.
How surgeons calculate the lifetime limit
- Map only stable skin. Leave out the nape, upper side fringes, and the occiput beside a thinning crown.
- Count follicular units per cm2 on the occipital midline and both parietal walls. Note hairs per unit and shaft caliber.
- Pick a single-pass density. At a baseline of 65 to 75, 10 to 15 extractions per cm2 is the usual safe band. Some experienced surgeons report routine use of 20 to 25, especially when density is above average and holes stay spread out.
- Subtract. Hold residual density at 40 to 50 follicular units per cm2, or higher if you wear a fade or have strong color contrast.
- Close a zone once it falls to about 60 percent of its own baseline. That stop rule is described by hairtransplantsource.com.
- Space the punches. Merged holes cause bare patches and can reduce local blood supply. No exact necrosis cutoff is proven, so do not cluster extractions.
Grafts do not regrow in the punch holes. Your donor is a budget you spend once, as drrevanthhairclinic.com notes. Thick, wavy hair hides more loss than fine, straight, high-contrast hair. Beard hair below the jawline can add grafts at a beard transplant clinic in Pune, but it does not raise the scalp limit.
Technique, cost, and what permanent means
Does hair transplant work? Yes, if the follicles are stable and the donor still covers. It will not stop loss in hair you did not move. Some people add PRP later. See PRP timeline notes.
Which hair transplant technique is best? The one that meets your goal without breaking the residual floor. FUE avoids a linear scar. FUT leaves a strip scar. DHI, direct implantation, and sapphire tools change placement or instruments, not your donor supply. After a follicular scan, iGraft usually plans ActiveFUE or DFI Ultimate.
A graft is one follicular unit, usually one to four hairs. Quotes that mix hairs and grafts are hard to audit. See graft versus follicle pricing.
ActiveFUE is ₹45 per graft. Gen3 DFI Ultimate is ₹50 per follicle. The cost of FUE in Pune, Bangalore, Hyderabad, or Vijayawada is that unit times a safe count, not a flat day rate. Check India pricing, Pune costs for 2,000 versus 3,000 grafts, Bangalore, Vijayawada, and the pricing table.
Which country is best for a hair transplant? About 30 percent of iGraft clients travel to India rather than Turkey for a physical consult and a unit quote. If you are searching for the best hair transplant clinic in Pune or Hyderabad, ask for baseline, excision density, and residual on paper. iGraft runs company-owned clinics, a setup of more than 15 years, in Pune, Bangalore, Hyderabad, and Vijayawada. You meet the doctor. See the about page.
Is FUE safe, and is the result permanent? It is generally safe when punches stay spaced inside the safe zone. Swelling, shock loss, uneven growth, and donor thinning remain possible. Follicles from a stable zone are long-lasting. Untreated hair can still thin. Success is not 100 percent. iGraft states a 29-year assurance on DFI Ultimate, which cannot cancel the donor ceiling. More is in myths about hair transplants.
When to book and when to wait
Book when the pattern is readable and other causes are checked. Age 18 or 20 is usually too soon. A completely bald person can be treated only if a permanent fringe remains, and advanced loss is often only partly coverable. Women are candidates when the donor is stable. Diabetes is not an automatic bar if sugar is controlled and your physician agrees. No month is medically best. Hot weather in India needs stricter sun and sweat care.
A full head shave is not always required. No-shave FUE suits some smaller sessions and takes longer. Surgery is usually one day under local anesthesia. Expect pressure, then soreness for a few days. Desk work often restarts within several days. Hard exercise and swimming commonly wait about two weeks. Most people judge the result between nine and twelve months. Another session needs at least six months, and only if residual density allows it.
FAQ
Is a hair transplant worth it?
Yes, when residual density still covers and the design fits your supply. A very low hairline that spends the whole donor is usually a poor trade.
How many grafts are needed?
Often 1,500 to 2,500 for a hairline, and more for a larger front and midscalp. Supply is safe zone area times density, minus the residual you must keep. Further passes are possible only while that residual holds.
What is the cost of hair transplant in India?
At iGraft, use ₹45 per graft for ActiveFUE or ₹50 per follicle for Gen3 DFI Ultimate. A 2,000 graft plan and a 3,000 graft plan are different bills.
Is hair transplant safe and permanent?
It is generally safe, not risk-free. Moved hair from a stable zone is long-lasting, not a cure for every thinning area, and not a guaranteed result.
Get a written density plan first
If a quote needs more than about 15 extractions per cm2 on an average donor, ask for the residual in writing.
Take the two-minute quiz, then meet a doctor in Pune, Bangalore, Hyderabad, or Vijayawada. Computerized checks and follicular scans should show baseline density, a safe excision range, and a unit price, with six months of follow-up after surgery. Start from the hair transplant page, the male hair transplant overview, or the home page.
