Finasteride After a Hair Transplant: The 48-Week Trial That Lifted Visible Improvement From 67% to 94%
Surgery moves hair. It does not stop the rest of your scalp from thinning. If you are booking a hair transplant in Bangalore, Pune, Hyderabad, or Vijayawada, the year-one look depends on graft handling and on the hair you leave behind. A 48-week trial is the clearest evidence that finasteride can change that second half.
What the 48-week trial measured
The study was randomised, double-blind, and placebo-controlled. It followed 79 men aged 20 to 45 with androgenetic alopecia who took finasteride 1 mg or placebo from four weeks before surgery until 48 weeks after it. At week 48, visible increases across the superior and frontal scalp were recorded in 94% of the finasteride group and 67% of the placebo group.
That 94% is a response rate, not a 94% rise in hair count and not a graft-survival rate. The trial scored hair surrounding the transplant.
Both groups improved. The extra gain was native hair that was less likely to keep miniaturising. The gap matters most if your recession is still moving. Long-stable loss is a weaker reason to start a daily drug.
A different paper, and a smaller gap
A separate prospective study followed 60 men, Norwood grades III to V, after follicular unit extraction. Thirty took oral finasteride 1 mg daily for 12 months. Thirty took no added medicine. At 12 months, mean graft survival was 94% versus 90%, and density rose by 28.6 hairs per square centimetre versus 24. Satisfaction averaged 8.6 versus 8.0 on a 0 to 10 scale. Two men (6.7%) had a temporary drop in libido that settled without stopping the drug.
The authors called the gaps significant. They also noted one centre, a modest sample, and only a year of follow-up. Do not treat a four-point survival gap as the 27-point visibility gap.
| Question | 48-week trial | 12-month comparison |
|---|---|---|
| Design | Randomised, placebo-controlled, 79 men | Prospective, 60 men, no medicine as control |
| Quoted figure | 94% vs 67% with a visible hair increase | Graft survival 94% vs 90% |
| What it does not prove | A 94% graft-survival rate | That every patient needs the drug |
What the tablet can and cannot do
Finasteride blocks 5-alpha reductase and lowers DHT in men with pattern hair loss, which can slow miniaturisation. It does not create follicles in a bald patch or design a hairline. A hair transplant does the placement. Medicine, if prescribed, tries to hold hair you did not move.
Guides that follow the product information say three to six months of daily use may pass before loss steadies. After you stop, gains start to reverse by about six months and move toward baseline by nine to twelve months. Repeat photos beat a daily mirror check.
Lower libido is the effect to raise first. It was uncommon and short-lived in the 60-man series, which is too small to call the drug risk-free. Do not borrow tablets or start them because a package listed aftercare.
The trial enrolled men only. Finasteride is not a default hair loss treatment for women, and pregnancy exposure can harm a male fetus. Do not handle broken tablets if you are pregnant or may become pregnant. Women need their own examination, including female hair transplant planning if surgery is even on the table.
Where this sits in an iGraft plan
iGraft Global Hair Services is a 15-plus-year, company-owned group in Pune, Bangalore, Hyderabad, and Vijayawada. Computerized scalp diagnostics and follicular-level scans separate active hair fall from baldness before a graft number is chosen. See how the clinics work.
ActiveFUE is ₹45 per graft. Gen3 DFI Ultimate is ₹50 per follicle. Confirm the unit before you compare hair restoration cost. Per-follicle pricing and graft versus follicle wording show how a quote can swell. Cost notes: India, Bangalore, Pune, and overseas quotes.
DFI hair restoration and ActiveFUE stop at donor limits and aim for a natural hairline design. Finasteride is not automatic. Young men with moving recession often need a medical plan if hair behind the new line is to hold. Stable loss may not. A men's hair restoration visit, including a male hair transplant consult, should end with that answer. About 30% of patients travel in from the US, the UK, Australia, and the NRI community for an in-person doctor and a published unit price.
Recovery is slower than the headline
Crusts settle over days to a few weeks. Desk work usually needs less time off than a physical job, which the days-off guide sets out. Shock shedding is common. Judge hair growth at 9 to 12 months, not week three. Three-week claims do not match the follicle cycle.
GFC hair treatment, PRP therapy, exosomes for hair, and BioCell treatment are adjuncts, sometimes with high-frequency care and low-level laser. They are not this trial and do not replace a prescription. Minoxidil is separate. Use the product page and the myths page before you treat a serum as surgery. Six months of support is the window to raise side effects or a slip in density behind the hairline.
FAQ
Do I need finasteride after every hair transplant?
No. The trial's extra gain was mainly in hair around the grafts. Stable loss may be surgery alone. Active pattern loss often needs both. Get a scan, not a blanket rule.
Does 94% mean the grafts survived?
No. In the 79-man trial, 94% of men on finasteride had a visible increase, versus 67% on placebo. A separate paper reported graft survival of 94% versus 90% at one year.
How soon will I see a change?
Not in the first fortnight. Finasteride may need three to six months before shedding steadies. Read the transplant at 9 to 12 months, with repeat photos.
What if I worry about sexual side effects?
Do not start the drug to fill a package. Two of 30 men in the small series had a short-lived libido drop. You can decline it if you accept that native hair may keep thinning.
Can women use the same 1 mg plan?
No. The trial was in men, and pregnancy exposure is a serious risk, including from broken tablets. Women need a separate diagnosis.
What is the graft cost at iGraft?
ActiveFUE is ₹45 per graft. Gen3 DFI Ultimate is ₹50 per follicle. Your total follows the units your donor and hairline need. If you are comparing a hair clinic near you in Vijayawada, Hyderabad, Pune, or Bangalore, get that unit in writing. Patient notes: Vijayawada, Pune, Bangalore, Hyderabad.
Get the scan before you add the tablet
Bring old photos if you have them. Start with the 2-minute hair loss quiz, then see a doctor at iGraft in Pune, Bangalore, Hyderabad, or Vijayawada. Leave with a graft estimate, a unit price, and a clear yes or no on finasteride.
